This chapter reviews the bony and ligamentous anatomy of the elbow, the physical exam findins associated with posterolateral rotatory instability as well as surgical treatment considerations. Finally, a minimally invasive technique for reconstruction of the lateral ulnar collateral ligament (LUCL) is presented which can be used in cases of isolated posterolateral instability (PLRI) or more severe elbow injurys and may obviate the need for hinged external fixation.

Surgical Repair of Posterolateral Rotatory Instability of the Elbow Using Surgilig/LockDown: A Novel Surgical Technique

Posterolateral rotatory instability (PLRI) of the elbow is the most common type of chronic elbow instability and is principally due to lateral ulnar collateral ligament injury. We describe a novel surgical technique for the treatment of PLRI using the Surgilig/ LockDown. Using this tensioned double-braided polyester synthetic ligament to reconstruct the lateral ulnar collateral ligament it is p...

Direct repair for managing acute and chronic lateral ulnar collateral ligament disruptions.

PURPOSE Acute elbow injuries that disrupt the lateral ulnar collateral ligament and result in posterolateral rotatory instability usually require surgical treatment. The 2 technical options reported, direct repair and use of a palmaris longus tendon graft, have usually favored the use of the graft. To balance this emphasis, we report our experience with direct repair of the humeral origin in ca...

Ulnar collateral ligament injury in the overhead athlete: diagnosis and treatment.

The ulnar collateral ligament (UCL) is more commonly injured than previously suggested. Injury to this ligament can result in secondary symptoms and problems in other parts of the elbow, including the ulnar nerve, the flexor-pronator musculotendinous unit, the radiocapitellar joint, and the posterior compartment of the elbow, in addition to being a cause of loose bodies within the elbow. This a...

Lateral Ulnar Collateral Ligament Reconstruction for Posterolateral Rotatory Instability of the Elbow

Reconstruction of the lateral ulnar collateral ligament of the elbow is the primary treatment for recurrent symptomatic posterolateral rotatory instability. Although a number of lateral ulnar collateral ligament reconstruction techniques have been described, the docking technique has received general acceptance. In this technique, the graft is passed through a tunnel on the ulnar side and the 2...

Non-anatomical reconstruction of lateral ulnar collateral ligament of the elbow after tumor resection

We present the case of an 80-year-old man with a tumor recurrence on his right arm 6 years after initial treatment. The lateral aspect of the elbow joint, involving overlaying skin, muscles, tendons, joint capsule, lateral collateral ligament complex, the lateral 1/3 of the capitellum, and lateral epicondyle of humerus were excised in the tumor resection. Intraoperative assessment revealed mult...
